Medical transportation is not only limited to ambulettes and medivans. In many instances, ambulances are also used to meet the growing non-emergency transportation (NEMT) needs.

Ambulances, typically thought of only in regards to emergency transportation, are often enlisted to provide non-emergency transports in areas where there is a lack of adequate medivan services or areas where there is a failure in understanding of the convenience and cost-savings abilities of using ambulettes.

Unfortunately, many staff, facilities, and organizations fail to understand the versatility and cost saving opportunities that exist in using ambulettes versus using an ambulance. Typically, a NEMT being performed by an ambulette is, on average, 70 - 90 percent cheaper and more cost effective as compared to an ambulance performing the same transport of equal distance and dynamics.

The cause for this gross disparity in cost can be attributed to the simple overhead economics. The cost of vehicle, insurance, training, and equipment for an ambulance is considerably more as compared to that of an ambulette. Further an ambulance is always staffed by multiple certified EMT's; further driving up labor and training costs. Conversely, an ambulette can be staffed by a single licensed driver trained only in state and local mandated transportation requirements. But again, this is only feasible for NEMT exclusively.

In states, counties, facilities, and areas where more and more ambulettes are being called upon to meet the non-emergency transportation needs of the growing elderly and disabled, there is a greater reduction in financial costs to insurance companies, Medicaid and Social Services, brokers, hospitals, non for profit organizations, and the like. At a time when budgetary concerns are being critically tested, such savings in using more ambulettes for non-emergency transportation needs is not only sensible and practical, but it's critical. Rather than reimbursing an ambulance service thousands of dollars for a single non-emergency transport, the cost to private-paying clients and facilities alike for a comparable transport can literally be reduced to a hundred or so dollars!

Needless to say, as we move forward with ever growing medical and transportation needs, coupled with changes in health care practice and policies, the need for practical and cost effective non-emergency transportation solutions will become more apparent. The need for safe and adequate services will increase with a continued attention on overall cost of delivery of such services, hence, the growing opportunity of non-emergency transportation services.

Whether you're a new or experienced entrepreneur, building a NEMT business can be a great asset in your portfolio! It will also serve as an outstanding learning and networking opportunity as you will meet a wide range of clients, professionals and strategic partners.

However, a very important piece of counsel is that you, the entrepreneur, thoroughly consider all sources of industry-related information prior to spending any money or committing to anything. Wise and calculated entrepreneurs always invest in thorough research and investigate the industry before entering for which some of this research does indeed cost money. In fact, very little credible information can be found for free.

Those working to gain an advanced academic education or learning a specific skill or trade are required to invest time, money, and effort into such education. So why would it be different when it comes to learning the details of starting and building your medical transportation business or any other type of business?

What are proving to be much more common occurrences are prospective entrepreneurs blindly following what I categorize as "simpleton" advice; very generic, broad advice that is highly limited in credible substance. Unfortunately, such advice is readily offered online from people who have never owned, operated, or even worked for a non-emergency medical transportation company. However, such "simpleton" advice illustrates and supports the age-old success principle that "Free advice is always more expensive!"

The result of this "simpleton" advice is a growing number of entrepreneurs frustrated, disheartened, and confused. Again, this is not limited only to the medical transportation industry. Many times, as a result of following poor advice, these entrepreneurs waste more time and effort trying to un-confuse themselves. Misguided entrepreneurs have to correct the mistakes that they've been making and "fill the gaps" as a result of misinformation and inadequate detail.

Now, don't get me wrong. I'm definitely NOT belittling anyone's attempts at sharing information or helping others. In fact, I am a very strong advocate for the sharing of ideas, information, and creative thinking. But the problem is that although the internet is a wonderful and incredible resource and opportunity to share and disseminate information, there exists a great deal of convoluted and unending stream of inaccurate information.

Although the medical transportation industry is not a high tech business it is, none the less, more involved than simply getting a vehicle, some insurance, and business cards. Needless to say, starting and building your own thriving non-emergency medical transportation company consists of much more detail, preparation, licensing and registration.

But here's the morale of the story. The problem with much of the free information that is available online is either wrong, incredibly inaccurate, or altogether misleading. So, always consider what you pay for. Whether it's the medical transportation industry or some other industry, if the information you're gathering is free, it's probably going to cost you the most! It may not cost you the most in terms of money, but it can definitely cost you the most in terms of time and effort into further research and preparation.

One of the most common misconceptions regarding the non-emergency medical transportation industry is that you need to be Medicaid approved in order to operate. This could not be further from the truth. There are many ambulette services that only service Private-Pay and contracted work. In fact, for the first two years of my business, I was exclusively a Private-Pay transportation provider until I figured out how to gain my Medicaid Provider Number.

Under the Social Security Act, the Department of Social Services is mandated to reimburse medi-van services for the non-emergency medical transportation of approved Medicaid recipients. However, Medicaid reimbursements rates can vary significantly not only between states, but between counties. There is no one single universal rate of reimbursement.

Further, in some counties and regions across the United States, the Department of Social Services outsources their administrative responsibility to brokers. Rather than work directly with transportation providers themselves, the Department of Social Services will contract with these "middle men" to reduce the burden and hassle to Social Services. In turn, these brokers are responsible for organizing, contracting with, contacting, and coordinating essential medical transportation for Medicaid recipients.

As is typical with any form of brokerage system, the broker makes money by taking a percentage of each transport. For example, using fictitious easy numbers, if the Department of Social Services pays $50 for a one-way wheelchair transport, the money will go to the broker. The broker will pay you, the transportation provider, $40 and the broker keeps $10.

Now, what you do with Medicaid transports, either working through a broker or Social Services directly, has NO bearing on what you do with Private-Pay or contracted work. Once you've gained proper operating authority within your state, you're technically in business. You can begin soliciting facilities and customers alike to meet the transportation needs of Private-Pay clients as well as begin soliciting and negotiating contracted work.

Medicaid or brokers have no ability to grant you operating authority or have any influence in what you do with Private-Pay or contracted work. Again, there are many transportation providers across the country that performs no Medicaid transports. For them, the Medicaid reimbursement rates might be too low to make it profitable or they're simply too busy meeting the needs of their Private-Pay and/or contracted clients.

I typically use the analogy of bricks and mortar. Depending on where you live, either Private-Pay or Medicaid will be the bricks of your foundation and the other will serve as the motar that keeps generating further revenue and keeps your vehicles moving. Typically, Private-Pay clients will yield a higher profit margin. Thus, you will want to always focus on your Private-Pay clients and use Medicaid transports to keep your vehicles and drivers moving and generating additional sources of revenue.

As we all know, we are currently enduring a struggling economy that is accompanied by rising inflation and an unemployment rate that is consistently holding close to 10%. Elderly and disabled people will be ever more in need of medical attention and assistance as well as medical transportation to and from treatments and appointments. Furthermore, as hospitals continue to increase their outpatient procedures and treatments, the need for more and more non-emergency medical transportation services are on the rise all across the United States and into related industrial countries.

The entrepreneurial opportunities in the medical transportation industry are growing more and more unprecedented and offering an increase in profitable solutions for many motivated entrepreneurs. The reason for such profitability is because the transportation needs of the elderly and disabled are indeed wide and diverse.

Many seniors remain ambulatory and in need of only limited transportation assistance. Although many seniors may require more time in walking, sometimes with a cane or walker, the do remain relatively independent in terms of mobility. However, in time, more and more seniors and disabled are finding the need for wheelchair and non-emergency stretcher assistance warranted for both safety and convenience concerns.

There are now several players offering non-emergency medical transportation franchises. Entrepreneurs benefit with the franchise model as they can operate under an established brand and get advice and training to get started and run a business. If you do your homework, you will find that this business is not that difficult and it is possible to start up and thrive independently.

Choosing the Right Area

Give some thought to the demographics of the area where you are setting up your business. Look for statistics that prove that there are a good number of elderly, disabled or Medicaid citizens living nearby. Compile a list of hospitals, dialysis centers, retirement homes, assisted living centers and other relevant operations. You could even make initial contact with some of these organizations and find out about the transportation solutions that they currently have in place.

Research the Competition

Identify all of the major competitors that you will have in your area and evaluate them in terms of their strengths and weaknesses. You may decide to incorporate parts of their business model into your own business and reject other parts. Learn as much as you can about them by reading their websites and calling their offices to ask questions. You can even spend some time following around some of their vans in order to understand how they work and to find out exactly who their customers are.

Getting Started - Licenses and Permits

Regulations and requirements for medical transportation businesses vary from state to state so you should make enquiries at the local level to find out what your obligations are.

As you are in the business of transporting paying customers from one place to another you will need some kind of permit, just as a taxi driver does. Your application for this kind of license will be handled by transportation authorities at the state or local level. Due to the nature of this work, some states will also require businesses in this industry to be registered with local health authorities.

There may also be other paperwork that needs to be done such as registering your business name, getting a business license (possibly more if you serve multiple counties) and zoning considerations if you run a home business.

Vehicles and Equipment

It is common for medical transportation services to purchase regular vans, either new or used, and then have them fitted out to suit special needs clients. Business owners typically start out with one vehicle and gradually add to their fleet as their business grows.

The best vans for an ambulette have a high raised roof and doorways. Access is usually on the side of the vehicle but can also be at the rear.

Some operators still use manual lifts to help them get wheelchair passengers into and out of their vans. However, you should be able to provide a faster, more professional service if you have a modern hydraulic lift installed.

Once inside the van, wheelchairs can be fastened to various securing devices so that they don't move around during transit. Vehicles also require customized seatbelts for wheelchair bound passengers. You can also improve the quality of your service if you have some comforts like TV and air conditioning.

A decent sized van that is properly kitted out should be able to transport four wheelchair bound passengers at a time as well as have extra seats available for caregivers. Depending on your clients, you may also need a space in your van that will allow you to fit in a passenger that is confined to a stretcher.

Insurance Policies

To protect yourself from unforeseen events you will need to purchase a variety of insurance policies. A general liability insurance policy will ensure that you are covered in situations where your customers are injured or suffer losses while they are in your care. You will also need regular automotive insurance to protect yourself in cases where your vehicles are damaged, stolen or involved in traffic accidents.

Payment Options

Decide on how you want to collect payment for your services. You may have to invoice medical institutions on a monthly basis or you may need to process credit card payments for individual clients. If you service large clients like Medicaid then it may be as simple as receiving a check in the mail each month. Talk to some prospective clients and look at what their needs are. Find out what payment options your competitors are offering.

Service Hours

Give some thought to your operating hours. The best ambulette services in the market usually offer weekday service from early morning to late at night with decent hours on Saturdays too. As a sole operator you will be limited in the hours that you can take on. As you expand into a multi-driver operation you will be able to offer more flexible scheduling to clients.

Staffing Requirements

If you start out doing all the driving while also managing your business you will quickly get burnt out. In order to grow you need to step back and hire some drivers so that you can move into a purely management and marketing orientated role.

Staff should have a drivers license that allows them to take passengers and they may also be subject to certain standards or requirements at the local level. No matter what you should make sure that your staff are trained on how to do the practical side of the job. They need to know how to assist passengers and their caregivers as they get in and out of vehicles. They should also be instructed to smile and make pleasant conversation if the opportunity arises so that they help your brand to gain a positive reputation.

Marketing
